Erath Middle School’s Renaissance Committee is selling Homecoming T-shirts.

● Return order to student’s homeroom teacher.

● Form and payment must be enclosed in a labeled and sealed envelope with your child’s name and grade level.

● Orders may dropped off at EMS Office

● Cash or Check - make payable to Erath Middle School **PLEASE SEND CORRECT AMOUNT- NO CHANGE WILL BE GIVEN

● Design 1 is a royal blue shirt.

● Design 2 is a royal blue shirt.

● All proceeds benefit the cost of Renaissance Day.

● Order Due by Thursday, September 22, 2022
